Consignor Request Form

After your consignment period ends, you can request your payout and schedule a date to pick up any unsold items using the form below.

The consignment shop will notify you by email once your check has been mailed or your check/items are ready for 

pick-up.

Important Reminders:

• Wait until your consignment period has ended to request your payout.
• Please cash or deposit checks promptly upon receipt; checks expire after 7 days.
• Requests to pick up unsold items must be submitted within 14 days after the consignment period ends.
