Consignor Request Form

• After your consignment period ends, you can request your payout and schedule to pick up any unsold items using the form below.

The consignment shop will notify you by email once your check has been mailed or your check/items are ready for 

pick-up.

Important Reminders:

• Wait until your consignment period has ended to request your payout.
• Please cash or deposit checks promptly upon receipt; checks expire after 7 days.
• Requests to pick up unsold items must be submitted within 14 days after the consignment period ends.
